PF-Atlas › CLL Treatments
CLL Treatment Options
Chronic lymphocytic leukemia is treated with targeted regimens rather than traditional chemotherapy in most current guidance. Fixed-duration venetoclax plus obinutuzumab and continuous BTK inhibitors are the leading first-line options. This page ranks the regimens PF-Atlas tracks by evidence and links their basis. It does not recommend any regimen.
How PF-Atlas grades evidence
Clinically established. FDA approved or positive Phase III.
Clinical research. In human clinical trials (Phase I to III).
Research model. Computational or preclinical hypothesis, not clinically validated.
Evidence-ranked regimens
Regimens are ordered by the PF-Atlas evidence score. Level 1 reflects randomized trial support. Choice of regimen depends on genetic markers such as del(17p) and TP53, fitness, and prior treatment, and is decided by a hematologist-oncologist.

Frequently asked questions
What is the first-line treatment for CLL in 2026?
Common first-line options are fixed-duration venetoclax plus obinutuzumab and continuous second-generation BTK inhibitors such as acalabrutinib or zanubrutinib. The choice depends on genetic markers, fitness and patient preference, and is made by a hematologist-oncologist.
Is chemotherapy still used for CLL?
Chemoimmunotherapy is used less often as targeted regimens have shown strong results in trials. Some regimens such as bendamustine plus rituximab remain relevant in specific situations. A specialist decides based on the individual case.
What is uMRD in CLL?
uMRD means undetectable minimal residual disease, a deep response where leukemic cells fall below a defined threshold. Fixed-duration venetoclax combinations are associated with high uMRD rates in trials. It is a research and prognostic measure, not a guarantee of cure.
How does PF-Atlas rank CLL regimens?
Each regimen carries an evidence tier based on trial support. Level 1 reflects randomized controlled trials. The ordering is a research summary, not a treatment recommendation.
Is this page medical advice?
No. PF-Atlas is a research atlas. CLL treatment is a medical decision made with a qualified hematologist-oncologist.
